if you are a member of another H.O.G. Chapter and are attending a Sydney H.O.G. Chapter ride, scroll to the QR code for the Chapter's visiting member form to help the Chapter manage ride safety.

If you are attending a Sydney H.O.G. Chapter ride but are not a member of H.O.G. please scan the appropriate QR code for an Adult or Minor participant, below, and complete the release form.

 

Visiting H.O.G. member form

This form is required for visiting H.O.G. members. It allows visitors to supply contact details to help the Chapter manage a safe ride.

Adult Chapter Release Form

This form is mandatory for adult non-members of H.O.G. participating in a Chapter ride.

Minor Chapter Release Form

This form is mandatory for minor (under 18) non-members of H.O.G. participating in a Chapter ride.
